Spiritual Care

Passionate Heart Hospice provides spiritual care counseling in keeping with the patient’s and family/caregiver’s belief system and practice, and in accordance with the plan of care.
The hospice chaplain (or designated spiritual care counselor) will provide direct support and coordinate services utilizing local clergy and other individuals, as appropriate.
A chaplain will be assigned to each patient based on the patient’s needs and acceptance of these services. The chaplain is a member interdisciplinary group and will participate in the development, implementation and revision of the plan of care.
